The Wilkes University athletic training staff is dedicated to providing quality health care in order to protect and enhance the health, safety, and welfare of each student-athlete at Wilkes University. All members of the athletic training staff are responsible for the prevention, evaluation, management, rehabilitation and treatment of injuries/illnesses that may occur during participation in athletics. The athletic training staff works in conjunction with Geisinger Health System team physicians, the University Health Services staff, as well as surrounding medical facilities in order to provide the best care for our athletes.

Wilkes University Student-Athlete Pre-Participation Forms
Please carefully review the information below regarding required medical forms. You will not be cleared to participate if any of the information is incomplete and/or missing. The Wilkes University Athletic Training Staff will strictly enforce this policy.
University Health Form
Upon enrollment, all students are sent instructions on how to submit the Wilkes University Health Form through the student portal. Student-athletes are only required one physical form for both the school and athletics. The section labeled "Recommendations for Physical Activity" must be completed by the practitioner in order to be eligible for participation in athletics.Â
Returning student-athletes who have participated in Wilkes University Athletics and who already have a physical on file do not need to have another physical. Although one initial physical is only necessary upon enrollment, it is recommended that all student-athletes have an annual completed physical done by their family physician.Â
Please note that Wilkes University does not perform pre-participation physicals on campus.

Athletic Training Forms
All new and returning Wilkes University student-athletes are required to complete the appropriate paperwork in order to be medically eligible to participate in intercollegiate athletics. All Wilkes University Athletic Training medical forms can be accessed, completed, and submitted online using Healthy Roster.Â
Student-athletes will receive instructions on how to create a Healthy Roster account and access the form(s) that must be completed and submitted prior to participation in any intercollegiate athletic practices, games, or out-of-season conditioning activities. Any student-athlete under the age of 18 must have accompanying parent/guardian signatures, when applicable.Â
All forms for freshmen, transfers, first-time student-athletes, and returning student-athletes must be submitted by the deadline. Student-athletes are encouraged to check their wilkes.edu e-mail accounts regularly for updates and deadlines regarding pre-participation paperwork.
All Athletic Training forms MUST be submitted electronically through Healthy Roster. Paper forms will not be accepted.

The NCAA and Wilkes University rules and policies require that the following forms must be completed and submitted before a student-athlete can participate in intercollegiate athletics:

Additional Notes
- If you had an injury/illness that prevented you from competing in part or all of last season/summer, you must submit a WRITTEN CLEARANCE from the attending physician (MD or DO) in order for you to participate.
