Required Paperwork for Freshmen, Transfers, & New Student-Athletes
The NCAA and Wilkes University policies require that the following forms must be completed and submitted before a student-athlete can participate in intercollegiate athletics.
Submit to University Health and Wellness Services (Passan Hall)

Upon enrollment, all students are sent instructions on how to submit the Wilkes University Health Form through the student portal. Student-athletes are only required one physical form for both the school and athletics. The section labeled "Recommendations for Physical Activity" must be completed by the practitioner in order to be eligible for participation in athletics. Returning student-athletes who have participated in Wilkes University Athletics and who already have a physical on file do not need to have another physical. Although one initial physical is only necessary upon enrollment, it is recommended that all student-athletes have an annual completed physical done by their family physician. Please note that Wilkes University does not perform pre-participation physicals on campus.
The University Health Form can be accessed by logging in to the Wilkes University Student Health Portal under "Health and Wellness Services" or by following the link below:

  2.   Detailed Medical History Form
  3.   Sickle Cell Trait Verification Form & Test Results
  4.   Copy of the front and back of insurance card(s)
  5.   Student-Athlete Contractual Agreements
The Wilkes University Sports Medicine Staff utilizes Healthy Roster as its electronic medical records system. As an incoming student-athlete, you will receive an e-mail invitation to your wilkes.edu address with "Join Wilkes on Healthy Roster" as the subject line. Follow the instructions below to learn how to set up your account. Please complete this process by August 1st. Your eligibility to participate is contingent upon completion of all required medical forms, which you will submit via Healthy Roster.
STEP 1. Open the e-mail invitation from Healthy Roster and use the link to sign up via the website (circled in red below).Â
Please note that you will be unable to create your full account through the app.Â
You must login through the website to enter all required information in order to be eligible for participation.Â
STEP 2. Follow the instructions to create your account.Â
Once your account is created, you will be directed to your Healthy Roster Dashboard.Â
Click on the ellipses button and "Edit Details" to complete your account information (circled in red below).
STEP 3. Complete ALL applicable information on the "Edit Athlete" page.Â
Fields marked with red asterisks are REQUIRED.Â
You will not be cleared to participate until all required fields are completed.Â
Click "Save Athlete" to save your account.
STEP 4: Submit ALL required forms under the "Documents" tab.Â
An Athletic Trainer will review and accept/reject your submitted document(s).Â
You will receive daily reminders from Healthy Roster until all documents have been submitted. Â
Additional Notes
  - If you had an injury/illness that prevented you from competing in part or all of last season/summer, you must submit a WRITTEN CLEARANCE from the attending physician (MD or DO) in order for you to be cleared to participate.
  - All Athletic Training forms MUST be submitted electronically through Healthy Roster. Paper forms will not be accepted.
